The Department of Health and Ageing is seeking applications for funding to build the capacity of the health sector to deliver improved health outcomes for Aboriginal and Torres Strait Islander people.

Funding will be provided to organisations to deliver outreach health services for Aboriginal and Torres Strait Islander people to address health issues identified in urban regional rural and remote locations including: Diabetes ,Cardiovascular disease, Chronic respiratory disease,Chronic renal (kidney) disease and Cancer.
Organisations are invited to apply for funding under the Aboriginal and Torres Strait Islander Chronic Disease Fund
Funding will be available for periods of up to three years,generally starting from July 2013
